Course Work Evaluation Request

Instructions for completion of the attached document:

  1. Complete the attached evaluation form. (PDF)
  2. Attach copies of all college transcripts that pertain to the course(s) to be evaluated.
  3. Attach copies of all course descriptions and/or syllabus of course(s) to be evaluated.
  4. Mail all documents to the following address:

                              Hill College
                              Health Science Department
                              2112 Mayfield Parkway
                             Cleburne TX 76033

The prospective student will be notified by phone or email when the evaluation process is completed.

It is important that the prospective student maintain an open line of communication with the Health Science department in this matter until the process is completed.

Failure to submit all required documents will delay the evaluation process and may result in a delay in completion of the students selected degree plan.

Hill College Enrollment Management will make every attempt to process this course evaluation within a reasonable time. The prospective student needs to be aware that there are peak times within the year that could delay the process for the requested course evaluations. The peak times could include, but are not limited to, the weeks before/during/after scheduled college registration and the weeks preceding the end of each semester.

The prospective student should not expect an immediate response when submitting the attached evaluation. If the prospective student is relying on this process to determine if he/she registers for a particular class; this form must be submitted well in advance of the scheduled registration date of the particular semester that the prospective student plans to enroll in.
